Lifetime risk for patients with mitral valve prolapse of developing severe valve regurgitation requiring surgery.

چکیده
Severe mitral regurgitation requiring surgery is the most common life-threatening complication of mitral valve prolapse (MVP) and is due to progressive myxomatous change in the valve. We identified all residents of New South Wales, Australia, who had mitral valve surgery for myxomatous valve disease during 1982 and, using these data and the adult population statistics from 1982, estimated the cumulative risk of valve surgery in patients with MVP. In 1982, 50 of the 5.36 million New South Wales residents required surgery for this complication of MVP. Of the 50, 36 were men and 14 were women, which was significantly different from the population sex distribution (p less than 0.02) for mean age +/- SD of 60 +/- 11 years (range, 26-78 years). Using our previously determined 4% prevalence of adult MVP in New South Wales, we estimated the number of male and female patients with MVP at risk for each 5-year age interval and calculated age-specific event rates. The results show that the cumulative risk is minimal below the age of 50 years but then rises steeply, particularly in men. The risks in men aged 50, 60, and 70 years (with 95% confidence intervals) were 1:202 (130-448), 1:53 (37-82), and 1:28 (22-41), respectively. In women, the risk was less than half that in men (p less than 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)

High-risk Coronary Artery Bypass Grafting and Mitral Valve Replacement in a HIV Positive Patient
Certain subsets of high-risk mitral valve patients are not suitable candidates for transcatheter therapies. The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...
متن کاملRight sided heart evaluation after successful mitral valve replacement.
Introduction: It is well-documented that right-sided heart dysfunction and significant tricuspid valve regurgitation (TVR) have adverse effects on patient outcomes after left-sided heart valve surgery. Therefore, the evaluation of right ventriclular (RV) function and TR severity in patients who had undergone mitral valve replacement (MVR), associated with/without concomitant su...

ABSTRACT: A consecutive Series of 522 patients (mean age 40.36 years ,female 70.9% ,male 29.1% ) with rheumatic valvular heart disease were studied during a period of seven years ,with particular reference to understanding the prevalence and risk factors of atrial fibrillation (AF) and systemic arterial thromboembolism (SAT). AF was observerd in 165 (31.6%) patients .The frequency of AF were ...

BaIlon mitral valvuloplasty and closed mitral valve conunissurotomy were performed in 450 and 127 patients, respectively. Also, 254 patients with severe mitral stenosis were enrolled in a prospective randomized trial comparing the two procedures. Immediately after balloon mitral valvuloplasty, mean left atrial pressure was 11.38±3.54 mmHg, transmitral valve gradient was 1.8±2 mmHg, and mit...
متن کاملEffect of position on echocardiographic diagnosis of mitral valve prolapse
Echocardiography has been accepted as the diagnostic modality of choice for mitral valve prolapse (MVP). The determine the effect of position on echocardiographic diagnosis of MVP, we examined 693 consecutive patients referred to the echocardiography laboratory because of suspected MVP. First, patients were examined using the usual standard M-Mode echocardiography( left parasternal third or fou...
